Apply Now for the Complex Needs in Medi-Cal Learning Network

HASC members, learn how to speed your Medi-Cal transformation and improve care for complex populations at Complex Needs in Medi-Cal: A Learning Network for Hospitals and Health Systems. Apply by Friday, Jan. 16, for the free, six-month opportunity, which starts in Feb. 2026.

Through monthly content, targeted coaching and shared learning, you’ll gain knowledge and tools that enable your hospital to improve care for high-need Medi-Cal patients, build community partnerships and revenue streams and enhance efficiency and resource utilization.

The program is designed for hospital leaders and staff in care management, population health, community health, data, performance and quality improvement and CalAIM.

Structure

Phase 1: Hospitals will join content calls and one of two cohorts for focused coaching, based on their current state and needs: 1) Contracted provider of ECM and/or Community Support services; or 2) Non-contracted provider in a referral role into Medi-Cal services.

  • Six Monthly Content Calls (1 hour): National best practices, case examples and facilitated discussions.
  • Five Monthly Cohort Coaching Calls (1 hour): Raise questions, share best practices and participate in peer coaching activities.
  • Two Quarterly Storyboards: Participating hospitals will update an easy storyboard PowerPoint template to outline their learnings and progress.
  • Ending Celebration Call: Harvest learnings and celebrate hospitals’ progress; open to colleagues.

Phase 2: In this optional phase, the cohorts will focus on emerging needs determined from Phase 1.

What You’ll Gain

The Complex Needs in Medi-Cal Learning Network aims to help your organization thrive amid changes and challenges. In Phase 1, participating hospitals will gain:

  • Actionable strategies to strengthen CalAIM implementation and performance: Build and refine your CalAIM infrastructure to improve efficiency, optimize resources and deliver better outcomes for Medi-Cal members, especially those with complex needs.
  • Sustainability tools to support high-need Medi-Cal patients amid financial and policy shifts: Develop resilient population health strategies and sustainable business models that align with Medi-Cal reforms and position your hospital for long-term success, even in times of uncertainty.
  • Data capacity to drive measurable impact: Strengthen your team’s ability to collect, interpret and use data that matters, e.g., ECM enrollment, referral patterns, ED utilization and outcome tracking, to demonstrate value and guide future investments.
  • Learning and cross-system insights: Practical, real-world lessons from peer hospitals on what’s working, overcoming shared barriers and accelerating collective progress in Medi-Cal transformation.

The series is hosted by HC2 Strategies and funded by the California Health Care Foundation.
