Communities Lifting Communities (CLC) is a 501(c)(3) nonprofit affiliate of HASC. The organization focuses on reducing health disparities and improving community health in HASC’s region of Los Angeles, Orange, Riverside, San Bernardino, Santa Barbara and Ventura counties.

CLC works to advance significant systems change through a collective impact model involving hospitals and health systems, public health departments, community clinics, Medi-Cal managed care plans and community stakeholders.

CLC receives funding from HASC member hospitals; AllHealth, Inc.; grants from Blue Shield of California Foundation and The California Endowment; and sponsorship from Health Net.

National Health Foundation (NHF)

National Health Foundation’s mission is to improve the health of individuals and underresourced communities by taking action on the social determinants of health. NHF’s vision is that all people, regardless of who they are or where they live, can achieve their highest level of health. 

Founded by HASC in 1973, NHF is a nonprofit that works within underserved Southern California communities to identify and advance solutions that remove barriers and promote health for all. The organization embraces the philosophy that health begins where we live, learn, work and play.

A community’s health is determined in part by access to social and economic opportunities; the resources and support available in our homes, neighborhoods and communities; the quality of our schooling; the safety of our workplaces; the cleanliness of our water, food, and air; and our social interactions and relationships.

NHF works hand-in-hand with communities to improve the conditions in which we live and right the wrongs of health disparities. It focuses on empowering people to make choices that allow them to live a long, healthy life, regardless of income, education or ethnic background.

NHF connects existing resources and ensures sustainability in programs so it’s not the sole champion of an effort. It also works to develop young leaders, change the physical and social landscape of vulnerable neighborhoods, and lift up communities. 

Using these strategies, NHF creates and deploys critical programs that are changing the way neighborhoods access healthy foods; enabling individuals to safely exercise without using their financial resources; shifting the level of expectations teen mothers set for themselves, their academics and their futures; and improving people’s chances of staying healthy by connecting them to homes.
