Voluntary data submission for the Agency for Healthcare Research and Quality’s (AHRQ) Surveys on Patient Safety Culture® (SOPS®) Hospital Database will be open June 1 to 19, 2026. Hospitals that have administered AHRQ’s SOPS Hospital Survey 2.0 any time between July 1, 2024, and June 19, 2026, may submit their most recent data.
Participating hospitals will receive reports comparing their data with aggregated, de-identified results from all contributors.
The SOPS Hospital Database will accept data from the following supplemental items along with the SOPS Hospital Survey 2.0:
- Health IT Patient Safety Supplemental Items
- Workplace Safety Supplemental Items
- Value and Efficiency Supplemental Items
