Communities Lifting Communities (CLC) is a nonprofit affiliate of HASC. CLC focuses on reducing health disparities and improving community health in the HASC region of Los Angeles, Orange, Riverside, San Bernardino, Santa Barbara and Ventura counties.
CLC works to advance significant systems change through a collective impact model involving hospitals and health systems, public health departments, community clinics, Medi-Cal managed care plans and community stakeholders.
Funding
CLC receives funding from HASC member hospitals, AllHealth, grants from Blue Shield of California Foundation and The California Endowment, and sponsorship from Health Net.
