CDIP Intern Testimonial Form

I agree that the Hospital Association of Southern California and its employees, agents, successors and assigns (collectively, “HASC”) may use my photographs and written and/or oral statements in connection with the College to Career Diversity Internship Program. (The term “photograph” or “photographs” used herein includes video or still images, in digital or any other format, and any other means of reproducing images. The same terms apply to written and oral statements, also referred to as quotes.) I hereby grant to HASC universal and perpetual rights to use and/or publish my name and/or likeness from these photographs in whole or in part, in any character or form, for marketing, promotion and any other lawful purpose, in any media currently or not yet existing, including but not limited to digital content. The same terms apply to oral or written statements or quotes. By virtue of the terms of this consent and release, I waive any right to inspect and/or approve the finished materials produced in connection with this release. I also waive any right to maintain any cause of action or claim against HASC or anyone else for the taking or use of my photographs or statements.
